Enrollment Enrolling is easy and available 24 hours a day via Self Service at http://leo.cc.emory.edu . How to Enroll Online Read this guide to decide which benefi ts will best meet your needs. Enroll online through Self Service at http://leo.cc.emory.edu . Enrolling online will ensure accuracy and save time. Current Employees Attend a Benefi ts Annual Enrollment meeting if you have questions or would like more information about Emory’s medical plans. To fi nd a meeting that is convenient for you, visit www.hr.emory.edu/ mybenefi ts , review the Take Care — What’s Changing for 2012 newsletter mailed to your home or check with the Benefi ts Department at 404-727-7613. 1. On the Self Service page, click Benefi ts, then click Benefi ts Enrollment. 2. Choose Select to view your current elections and the new election options. 3. Complete certifi cations — Spouse/SSDP and Tobacco 4. Change your elections by clicking Edit beside a particular plan and follow the prompts to view and/or change your current coverage. You may also enroll, add or drop dependents. 5. After editing your current coverage in each plan, your new elections, covered dependents and payroll deductions will be displayed. 6. You will be prompted at the bottom of the page to Continue if coverage is correct or Edit if you have made an error. 7. Print a copy of the enrollment summary after you have edited and fi nalized your elections. 8. Click Continue at the bottom of the Enrollment Summary page and then Submit after reading the Authorize Elections Statement. Print the confi rmation page for your records. Note: You have not enrolled until you click Submit. Review your benefi t elections through Self Service at http://leo.cc.emory.edu . If you enroll during the Benefi ts Annual Enrollment period, you will receive a confi rmation statement of your benefi t elections at your home address in December. Please read the statement carefully to ensure accuracy as after January 1, 2012, most elections cannot be changed. New Hires You may enroll online anytime during your fi rst 31 days of employment. Be sure to complete your online orientation OR attend a benefi ts orientation (Webcast or In-Person) to learn more about benefi ts and receive important information, or contact the Benefi ts Department with your questions at 404-727-7613. Need Computer Access to Enroll? Employees have the ability to enroll online at any public access computer that has internet connectivity. To access Self Service ( http://leo.cc.emory.edu ), you will need your network ID and password. If you do not know your network ID or password, please call the UTS helpdesk at 404-727-7777 (available Monday – Friday, 7 a.m. -6 p.m.). Employees can also access online enrollment via a kiosk at the campus locations listed below: • FM Building C Breakroom • DUC Lobby • Benefi ts Department, 1599 Clifton Road, 1st fl oor • Cox Hall • White Hall Reminder If you are electing medical coverage you must complete the following: 1.
